Dr. Feldman's dimensional planning uses a data driven, analytical approach to select implants based your chest and breast measurements with adjustments for your anatomical asymmetry that therefore will always be proportionate to your frame.
Breast implant selection must be based on a detailed exam and measurements of your breasts, not the number of cc’s that your friend or family picked, desired cup size, or what looks good on our website photo gallery. Dr. Feldman wishes it was that simple! The single largest influence on what your breasts look like after breast augmentation surgery, is what your breasts look like before surgery.
It's a constant battle in our practice attempting to refocus patients away from random lucky numbers of cc's and toward a pleasing breast aesthetic. Will you be satisfied with an ugly result just because it falls within an arbitrary cc range? Not a chance. Will you be OK with your desired cup size even with wide cleavage, nipple malposition and no superior fullness? No way. During your consultation and pre-operative visit you were repeatedly reminded to focus the look, not the cc’s or cup size. If you love your result, in 6 months you won’t remember the number of cc’s, and if you picked a random lucky number and had a bad result, you would no longer care about that number. Breast aesthetics are far more complex than just a number of cc's or cup size. In fact, Dr. Feldman never lets his patients focus on, or promises, a specific number of cc's as it's only a setup for disappointment.
It’s not what Dr. Feldman puts in, but rather how pretty your breasts are when you come out of the OR that matters.
Hope is not a surgical technique nor sizing method. Don't trust a surgeon who didn't take and explain measurments themselves. Don't accept a staff member or computer.
To better understand what is going on in Dr. Feldman's mind during your exam and implant sizing, review some basic engineering, geometry and math terminology he frequently discusses during your consult here.
Implants are manufactured in multiple styles or profiles (moderate, mod+, full, high or ultrahigh). The breast implant profile selected will control the look (meaning more or less natural) as well as the size. Within each style, only 1 or 2 implant sizes will match your dimensions close enough to be considered “dimensionally sound.” Once the width of your current breast is measured, and any adjustment to that number is made for narrow or wide cleavage, absent or excessive lateral show (side boob) “dimensionally sound” implants can be selected. Among different breast implant profiles the number of cc’s will not be the same, as to maintain the same width, a higher profile implant will be a greater number of cc’s as it has more projection (sticks out father from your chest).
In general, in Dr. Feldman’s experience, a dimensionally sound moderate plus profile (Allergan Natrelle Inspira SRM) tends to go from an A+/B- pre-op to the magical full C or small D after breast augmentation surgery meaning it adds about 2 cup sizes, and provide what he calls the “Are they real or not?” look. A full profile (Allergan Natrelle Inspira SRF) would be about 100ccs larger to maintain the same base width and is more like a Full D but Victoria’s Secret will call it a DD or DDD meaning it adds about 3 cup sizes, and yields a “New Normal” or "push up bra" look with more cleavage "pop" and more projection. The Allergan Inspira line area the same gel and shell as the classic Natrelle implants with a slightly higher fill ratio, yielding more durable superior fullness and less rippling and over 95% of Dr. Feldman's patients select Inspira implants. Ultimately, when you try on the different breast implant styles during your consult and look in the mirror, you will quickly fall in love with one style or the other!
Certain breasts or anatomy respond better and have prettier results with certain styles, so Dr. Feldman may recommend a particular style, and if he is confident a specific style will not look good, it will not be offered.
